Pancreatic carcinoma is one of the most aggressive cancers in gastroenterology, an important cause of mortality and morbidity. The molecular mechanisms of this aggressive behavior were not elucidated, although pancreatic cancer is characterized by a variety of molecular alterations and over-expression of several mitogenic and angiogenic growths factors and their receptors. The use of minimal invasive imaging procedures will pave the way for real-time assessment of key biomarkers of angiogenesis, as well as pancreatic cancer stem cells. Two newer imaging approaches used for the real-time in-vivo evaluation of angiogenesis will be tested during the project, including contrast enhanced endoscopic ultrasound, as well as confocal laser endomicroscopy micro-fibers. Clinical protocols will include harvesting of pancreatic cancer tissue through endoscopic ultrasound-guided procedures and/or surgery, followed by evaluation of tumoral microvessel density and other markers of angiogenesis (including the VEGF pathway) using state-of-the-art IHC methods. Gene expression by qRT-PCR will be used to investigate the genes involved in the modulation of biological processes of angiogenesis. Finally, the application of modern biostatistical and machine learning techniques will be assessed.
